29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) On or about July 6, 2025, at the jobsite, employees used an electric circular saw without eye protection while performing framing for a window.

29 CFR  1926.300(d)(3):Other hand-held powered tools, such as circular saws, chain saws, and percussion tools without positive accessory holding means, were not equipped with a constant pressure switch that would shut off the power when the pressure was on.  On or about July 6, 2025, at the jobsite, an employee used an electrical circular saw without a  constant pressure safety switch.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  On or about July 6, 2025, at a residential dwelling,  where a work related in-patient hospitalization of an employee that sustained amputations of the left hand's index and middle fingers was not reported to OSHA within 24 hours.
