OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from the hazard of exposure to carbon monoxide while operating a gasoline-powered generator in an enclosed section of a mobile pet grooming van.  a) On or about August 02, 2025, jobsite located at 2631 NE 20th Ave, Lighthouse Point, FL 33064, an employee was exposed to carbon monoxide poisoning due to the operation of a Westinghouse iGen4500 (s/n 45A0622B4167) generator in an enclosed section of a mobile pet grooming van.

29 CFR 1910.1000(a)(2): Employees were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8-hour Time-Weighted Average concentration of 50 ppm:  a) On or about August 02, 2025, jobsite located at 2631 NE 20th Ave, Lighthouse Point, FL 33064, an employee was exposed to an 8-hour time-weighted average (TWA) of 186.1 ppm of carbon monoxide when working inside a pet grooming van using a Westinghouse iGen4500 (s/n 45A0622B4167) generator.
