29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about August 5, 2025, the employer failed to ensure that employees used appropriate eye or face protection such safety glasses when exposed to liquid chemicals. Employees in the cleaning department use chemicals to include, but not limited to, Goof Off.

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, was not provided with guard which was in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about August 5, 2025, the employer did not ensure that effective guarding was in place on the VLS660 Universal Laser Systems to protect employees from the point of operation. Employees are tasked with operating the equipment while the interlocked barrier guard is open allowing them to reach into the hazardous area during operation.
