29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) On or about August 5, 2025, at the jobsite: employees were exposed to eye injury hazards while using circular saws (DeWalt DWE575 Circular saw) to cut siding, in that safety glasses were not used.  b) On or about August 5, 2025, at the jobsite: employees were exposed to eye injury hazards while using nail guns (SENCO LHT-64 nail gun) to install siding, in that safety glasses were not used.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about August 5, 2025, at the jobsite: employees installing siding were exposed to a 9'4" fall hazard, in that the ladder (Little Giant Multi 18 Ladder Type 1A Part Number  16517-002) was set approximately 6" inches past the roof landing.
