29 CFR  1910.147(a)(3)(i):The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees:  a) Facility, 4646 Clear Shade Drive, Windber, PA:  On or about July 31, 2025, an employee suffered an injury while attempting to unlock brakes on a Peterbilt tractor after it had seized/frozen from sitting in the yard for several weeks.  The keys were is the ignition, there was air pressure in the braking system and the wheels were not chocked.
