29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Top girts on wall posts; On or about August 8, 2025, employees installing roof trusses were exposed to falls of approximately 18 feet to a lower level while straddling the top girts without the use of conventional fall protection (guardrails, safety nets, or personal fall arrest systems).

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Post-frame wall girts; On or about August 8, 2025, the employer did not ensure employees used the available ladders or personnel lifts at points of access with breaks in elevation of 19 inches or more, when employees accessed the top of an 18-foot wall by climbing the structural girts, which were spaced 24-inches or more, to set trusses at the top of the posts.
