(a) Worksite: On or about 8/12/2025, the employer did not ensure that employees were protected from struck by hazards when a septic tank baffle was not stored in a secured manner during transport to a worksite. The baffle section was stored in a vertical position and when the strap was released it tipped over and struck an employee.
