29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables were used:  On or about August 12th, 2025, employees engaged in residential roofing operations located at 3231 North 186th Plaza, building #1, Elkhorn, Nebraska 68022, were exposed to electrical shock and burn hazards in that the employer had the employees using a damaged flexible(extension) cord used to supply power to their microwave.  At the receptacle end of the cord the connecting point of the secondary hot wire was cut free from the interior insulation exposing the interior copper wiring.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about August 12th, 2025, employees engaged in residential roofing operations located at 3231 North 186th Plaza, building #1, Elkhorn, Nebraska 68022, were exposed to electrical shock and burn hazards in that the employer had the employees using a damaged flexible(extension) cord used to supply power to their microwave.  At the receptacle end of the cord the outer insulation designed to protect the interior wiring has been pulled back from the receptacle due to the lack of strain relief during use.  The result is the connecting point of the secondary hot wire was cut free from the interior insulation exposing the interior copper wiring.

On or about August 12th, 2025, it was observed that multiple employees engaged in residential roofing operations located at 3132 North 186th, building #1, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer had multiple employees  working at heights greater than 25 feet without any form of fall protection in place.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 12th, 2025, it was observed that employees engaged in residential roofing operations located at 3132 North 186th Plaza, building #1, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer had employees using an extension ladder for access and egress to and from the roof without ensuring the ladder was extend at least 3 feet above the upper landing surface.

29 CFR  1926.1053(b)(22):Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  On or about August 12th, 2025, it was observed that employees engaged in residential roofing operations located at 3132 North 186th Plaza, building #1, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer failed to ensure employees were not carrying bundles of shingles on their shoulders as they climbed an extension ladder set up over 25 feet ground to eave.
