29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) New residential construction located on 362 Red Petal Lane, Canton, GA 30114: On or about 08/12/2025, the employer exposed employees to fall hazards of approximately 14 feet, 2 inches to the lower ground level, in that a fall protection system was not provided to employees engaged in siding installation activities working from a pump jack scaffold system.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New residential construction located at 362 Red Petal Lane, Canton, GA 30114: On or about 08/12/2025, the employer failed to inspect all electrical extension cords used to provide power to electric tools, in that a 25 ft, yellow electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
