29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a) On or about 08/13/2025, throughout the establishment; Employees were exposed to caught-in hazards when performing servicing and/or maintenance activities such as, but not limited to: cleaning Mazak YMSH40-Q Serial number M72351 and  changing blades on the bandsaw. Employees were not trained as Authorized employees to recognize hazardous energy sources or use proper procedures to control those hazards.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  a) On or about 08/13/2025, in the H40Q area; employees were exposed to the unexpected startup of equipment when conduction servicing or maintenance activities on machine such as, but not limited to the Mazak YMSH40-Q Serial number M72351, and a group lock-out procedure was not followed to ensure every employee performing tasks applied their own lock to a group lockout device.   ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a) On or about 08/13/2025, throughout the production area; employees were exposed to fire hazards when directional exit signs were not posted directing employees to exit door locations.  Exit doors signs were not visible in production areas because they were located directly behind very large machines such as large boring mills.  ABATEMENT CERTIFICATION REQUIRED
