29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On August 14, 2025 at 1618 5 1/2 Ave Ne in Jamestown, ND, employees were exposed to a fall of greater than 6 feet while conducting roofing activities on a residential roof without the use of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) On August 14, 2025 at 1618 5 1/2 Avenue NE, Jamestown, ND, employees were exposed to hazards associated with the displacement of a ladder who's rails were not 3 feet above the upper landing surface while working on a roof.
