29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: a) Jobsite: 14515 McNulty Rd, Philadelphia, PA - On or about August 19, 2025, the employer did not ensure employees utilized personal protective equipment when handling chemicals during the construction of the concrete motor vehicle drain. Employees were handling a mixed concrete product from JFI Redi-Mix containing chemicals such as Tall Oil, Sodium Salt, and Tributyl Phosphate without the use of hand protection.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Jobsite: 14515 McNulty Rd, Philadelphia, PA - On or about August 19, 2025, the employer did not provide adequate hazardous chemical training for employees engaged in the construction of a concrete motor vehicle drain on the floor of the building. Employees were not adequately trained on the hazardous chemicals, including but not limited to Tall Oil, Sodium Salt, and Tributyl Phosphate contained in the JFI Redi-Mix, LLC Concrete Mix.
