29 CFR  1926.1412(a)(1): Equipment that has had modifications or additions which affect the safe operation of the equipment (such as modifications or additions involving a safety device or operational aid, critical part of a control system, power plant, braking system, load-sustaining structural components, load hook, or in-use operating mechanism) or capacity was not inspected by a qualified person after such modifications/additions were completed, prior to initial use:   On or about August 19, 2025; on the northwest side of the jobsite: employees utilized a crane to lift steel trusses without an inspection by a qualified person who inspected the addition of the cotter pin to the hook latch.

29 CFR 1926.1412(d)(1)(v): The employer did not inspect hooks and latches for deformation, cracks, excessive wear, or damage such as from chemicals or heat.  On or about August 19, 2025; on the northwest side of the jobsite: employees utilized a crane to lift steel trusses without an inspection on the latch for deformations, cracks, excessive wear, or damage such as from chemicals or heat.
