29 CFR 1910.29(b)(1): The employer did not ensure that  guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11 of this section).  On or about August 15, 2025, and at times prior thereto, at Machine 703, employee(s) utilized an elevated platform without adequate guardrails in place while performing copper coil resizing and coiling operations.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about August 20, 2025, and at times prior to, the employer did not notify OSHA within twenty-four (24) hours of an amputation caused by a work-related incident involving an employee, occurring on August 15, 2025.
