29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) On or about August 19, 2025, and at times prior to that date, the employer did not have a program in place to isolate and disable the power source to the conveyer system located throughout the facility, to include a means to physically lockout or isolate the power source, while employees are engaged in performing maintenance on the conveyer systems, exposing them to caught in-between hazards.
