29 CFR  1910.244(a)(2)(iii):Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack.  a) On or about August 22, 2025, and at times prior, the employer did not ensure loads were cribbed, blocked or otherwise secured after being raised with a Daytona floor jack when an employee was performing brake repairs on both the left and right side of a  Dodge Ram Promaster van, requiring him to place portions of his body including his head under the van on both the left and right side. The left side of the van was raised with a Dayton floor jack and only the right side had a jack stand to secure the load.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).  a) On or about August 22, 2025, and at times prior the employer did not ensure that inside the breaker panel in the saw area, that live parts of electrical equipment operating  at 50 volts or more were guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, there were open breaker sections inside the panel.  b) On or about August 22, 2025, and at times prior, the employer did not ensure the electrical boxes coming from the electrical panel with electrical wires were covered to prevent accidental contact.
