29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On August 25, 2025, employees performing framing activities were exposed to falls of approximately 10 feet while using a collapsed A-Frame ladder to access trusses. The ladder extended approximately 4 inches past the landing surface. (b) On August 25, 2025, employees performing framing activities were exposed to falls of approximately 10 feet while using a collapsed A-Frame ladder to access trusses. The top step of the ladder was approximately 1 foot 6 inches below the upper landing surface.
