29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about July 7, 2025, and at times prior thereto, employees were utilizing a zero-turn mower that was missing guarding to protect from nip points, rotating parts, and flying chips.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about July 8, 2025, an employee was hospitalized due to a workplace injury and the employer did not report the hospitalization within 24-hours.
