29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) Deutsche Roofing ComAg Systems LLC DBA DCA Roofing on or about August 27, 2025 at the location of 10540 Fremont Pike Perrysburg, Ohio 43551: The employer did not ensure the portable ladder use by employees to access/egress to and from the upper level was extended at least 3' above the landing surface. Employees using the ladder in this condition were exposed to fall hazards.
