29 CFR 1910.23(c)(11):  The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface:   On or about September 4, 2025, an employee was exposed to a fall hazard of approximately 20 feet while using a ladder to access the roof because the side rails did not extend at least three feet above the upper landing surface.

29 CFR 1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system:  On or about September 4, 2025, an employee was exposed to a fall hazard of up to 20 feet while on the roof installing holiday lighting because a suitable fall protection system was not used.
