(a) On or about September 4, 2025, at N53 W19798 Kingair Dr., Menomonee Falls, WI 53051, the employer did not ensure employees conducting roofing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

(a) On or about September 4, 2025, at N53 W19798 Kingair Dr., Menomonee Falls, WI 53051, employees installing a roof on a residential home roof with a 11:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 22 feet 2 inches from the roof eve to the ground below on the front side of the home.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about September 4, 2025, at N53 W19798 Kingair Dr., Menomonee Falls, WI 53051, an employee conducting roofing activities on a 11:12 pitch roof was not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.
