(a) On or about September 4, 2025, at N53 W19798 Kingair Dr., Menomonee Falls, WI 53051, an employee conducting roofing activities on a 11:12 pitch roof was not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the waist (belt area) and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.
