29 CFR  1910.28(b)(2)(i): The employer did not ensure that each employee in a hoist area is protected from falling 4 feet (1.2 m) or more to a lower level by one of the systems listed in 29 CFR 1910.28(b)(2)(i)(A)-(C).  a.) On or about September 5, 2025, on the mezzanine, employees were exposed to fall hazards of approximately ten feet (10')  when required to work near a hoist area that was not protected with fall protection.  The hoist area is composed of  six pallet drop-off sections, was not equipped with functional guardrails or equivalent safeguards.

Production Area- On or about September 9, 2025, the employer failed to provide effective guarding to protect employees working at Dressing Line 1 Filler. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.  b. Production Area- On or about September 9, 2025, the employer failed to provide effective guarding to protect employees working at Dressing Line 2 Filler. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.  c. Production Area- On or about September 9, 2025, the employer failed to provide effective guarding to protect employees working at  Dip Line 6 Filler . Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.

29 CFR 1910.219(i)(2):  Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve.   a.) On or about September 5, 2025, at the end of the dressing line, the employer failed to ensure that machine guarding was installed on the shaft coupling identified as #F-526, exposing employees to contact with the rotating coupler during normal production. Employees were thereby exposed to machine hazards due to the unguarded rotating shaft coupling.  b.) On or about September 5, 2025, at the conveyor tunnel dressing line exit L01, the employer failed to ensure machine guarding was installed on the shaft coupling (serial #39016), exposing employees to contact with the rotating coupler during normal production.  Employees were thereby exposed to machine hazards due to the unguarded rotating shaft coupling.  c.) On or about September 5, 2025, at the conveyor sleever entry L02, the employer failed to ensure machine guarding was installed on the shaft coupling (serial #39041), exposing employees to contact with the rotating coupler during normal production.  Employees were thereby exposed to machine hazards due to the unguarded rotating shaft coupling.
