29 CFR 1926.300(b)(1):  Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a)  On September 8, 2025, an employee was using unguarded Hilti angle grinder while performing masonry work.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  a) On September 8, 2025, employees working from frame scaffolding were exposed to fall hazards of greater than 25 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a)  On or about September 8, 2025, an extension ladder used by employees to gain access to the working surface did not have its side rails extended at least 3 feet above the surface.   No abatement certification or documentation is required for this item.
