29 CFR  1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) Guadalupe Roofing, LLC, failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1 Item 3 a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of February 26, 2026.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) On or about September 10, 2025, at the jobsite located at 130 Wisconsin Ave, Pewaukee WI, employees were installing shingles on a commercial roof with pneumatic nail guns without the use of eye protection.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:  (a) On or about September 10, 2025, at the job site located at 130 Wisconsin Ave, Pewaukee WI, employees were installing shingles without being protected from fall hazards of up to approximately 40 feet.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about September 10, 2025, at the job site located at 130 Wisconsin Ave, Pewaukee WI, employees who were installing shingles on a four story condo building without fall protection were not trained to recognize fall hazards and to implement fall protection methods.
