Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a) On or about 09/10/2025, the employer did not ensure that employees performing roofing activities on a steep roof commercial building at 2890 Mack Rd, Fairfield, OH 45014 were utilizing a fall protection system in that employees were not tied off to anchor points. Employees were exposed to a fall hazards greater than 16 feet on a slope that was on a 5/12 pitch.
