On or about September 12, 2025, an employee was exposed to fall hazards approximately 19 feet, 10 inches high while performing roofing operations on a residential property. The employee was observed not using fall protection correctly when the lifeline lanyard was connected to the front nylon straps while performing roofing operations.

On or about September 12, 2025, at the above addressed jobsite, employees performing residential roofing operations, approximately ten (10) feet six (6) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.
