29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) On or about September 12, 2025, on site of a residential structure located at 10291 Highpointe Drive in Stuart, Florida, an employee was exposed to an approximated 12-foot fall hazard when climbing up an extension ladder while carrying metal roof vents in his hand which could have caused the employee to lose his balance and fall.   b) On or about September 12, 2025, on site of a residential structure located at 10291 Highpointe Drive in Stuart, Florida, an employee was exposed to an approximated 12-foot fall hazard when climbing down an extension ladder while carrying a box of tin caps in his hand and on his shoulder which could have caused the employee to lose his balance and fall.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 12, 2025, on site of a residential structure located at 10291 Highpointe Drive in Stuart, Florida, an employee was exposed to a fall hazard of approximately 12 feet when working on the roof of the structure while not being protected from falling to the lower level.  All Phase Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1826196, citation number 1, item number 1, and was affirmed as a final order on December 2, 2025, with respect to a workplace located at 4345 Corner Stone Way in Stuart, Florida.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:   On or about September 12, 2025, on site of a residential structure located at 10291 Highpointe Drive in Stuart, Florida, employees performing roof work were exposed to a fall hazard of approximately 12 feet when utilizing a ladder prior to the employer providing them with training by a competent person for the proper use, set up and hazards associate with ladders.  All Phase Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1060(a)(1), which was contained in OSHA inspection number 1679850, citation number 1, item number 3, and was affirmed as a final order on January 23, 2024, with respect to a workplace located at 23094 Bel Conto Road in Boca Raton, Florida.
