29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. Location: 21 Curtis Path, East Northport, NY 11731 On or about 09/12/2025, employees were installing shingles on a single-story residential home while working on the roof approximately 14ft above the ground below. The employees were not protected by a means of fall protection and were exposed to falling to the ground below. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. Location: 21 Curtis Path, East Northport, NY 11731 On or about 09/12/2025, employees were accessing the roof of a residential home by using an aluminum extension ladder that was only extended 1.5ft above the landing. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
