On or about September 9th, 2025, and at times prior thereto, employees were tasked to mix sodium hypochlorite into 25-gallon freestanding vertical tanks to disinfect wastewater for drinking and other uses at the national park. The employer did not ensure employee(s) were provided an eyewash for quick drenching and/or flushing of eyes for 15 minutes due to unexpected exposure to chemicals and/or potential exposure within the employee(s) work area. This condition potentially exposed employee(s) to eye irritation, corneal abrasion, and/or serious eye damage. In the following location(s): a. South entrance - No plumbed eyewash station. b.
