29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  On or about May 16, 2025, at 2506 Mulberry Drive in Mansfield, Texas, employees performing masonry activities used a yellow extension cord that was severely cut and frayed, supplying power to a Makita right hand angle saw, exposing employees to electrical hazards.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  On or about May 16,  2025, at 2506 Mulberry Drive in Mansfield, Texas, the employer did not ensure that employees who used a Makita right hand angle saw to cut bricks/stone were protected from being exposed to silica dust with the use of engineering controls, work practices and respiratory protection.

29 CFR  1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:   On or about May 16,  2025, at 2506 Mulberry Drive in Mansfield, Texas, employees who were cutting masonry stone and brick with a Makita right hand angle grinder were not provided a respirator.
