29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   At the jobsite located at 15080 W154th Street, Olathe, KS, employees engaged in residential roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards, in that eye protection was not utilized.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the jobsite located at 15080 W154th Street, Olathe,  KS, employees engaged in residential roofing activities were exposed to fall hazards of approximately 21 feet from the eave to the ground below in that proper fall protection was not utilized. Two employees had climbing belts on which do not meet the standard.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  At the jobsite located at 15080 W154th Street, Olathe,  KS, employees were exposed to fall hazards of  approximately  12 feet due to several ladders not being properly secured to the gutter or at least three feet above the eave of the roof.
