29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a)   Roof - On or about September 10, 2025, and times prior thereto, the employer exposed employees to a fall hazard, in that fall protection was not used by employees that performed roofing work on a steep roof with a 30'8" fall to ground distance.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   (a) Job site - On or about August 27, 2025, the employer exposed employees to a fall hazard, in that each employee was not provided training to recognize and minimize hazards related to ladders prior to performing work from ladders.
