(b) On or about September 16, 2025, on the 4/12 pitch roof; employee(s) were performing shingle installation activities on the residential construction roof without adequate fall protection.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about September 16, 2025, on the 18/12 and 4/12 pitch roof; employees were not trained to recognize fall hazards while performing shingle installation activities on the residential construction roof.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
