29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Residential structure, roof deck; On or about September 15, 2025, and September 16, 2025, one or more methods of fall protection was not in place while a worker performed sheeting activities at heights ranging between approximately 11-feet, 2-inches to 22-feet, 7-inches above ground level.  (b) Residential structure, main floor; On or about September 16, 2025,  workers were not protected from falling into a hole cut out for the basement stairway by one or more fall protection methods.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Residential structure, front entryway; On or about September 16, 2025, a multi?positional ladder configured as an extension ladder was set up at ground level and used by a worker to access the roof deck, approximately 11 feet 2 inches above the ground. The ladder's side rails did not extend at least 3 feet above the landing surface.
