29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about September 17, 2025, the employee working on the ground to perform cleanup and provide materials to assist the roofers who were working directly above while re-roofing the structure, was not protected from struck by hazards from falling materials and tools by wearing a protective helmet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 17, 2025, employees conducting roofing activities on a residential home structure to include tear-off, and shingle installation were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to a fall hazard of 10-feet to16-feet onto concrete and grass.
