29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about September 18, 2025, Hostetler Truss did not  establish a energy control program consisting of energy control procedures, employee training and periodic inspections.  Employees using machines to include but not limited to the Auto OMNI Saw, were not protected from the unexpected release of energy, including but not limited to, electrical and pneumatic energy, when changing saw blades.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  On or about September 18, 2025, Hostetler Truss did not ensure that employees performing service/maintenance on the Auto Omni Saw received training to ensure authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls when changing saw blades.

29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   On or about September 23, 2025, Hostetler Truss failed to ensure that employees operating forklifts including but not limited to Clark (diesel powered ) Forklift, Model Number CGP 30, Serial Number P365D-1132-9399FB), were fully trained prior to allowing them to lift pallets, skids and other materials in and around the facility.

29 CFR  1910.213(r)(4): The mention of specific machines in 29 CFR 1910.213(a) thru (q) and 29 CFR 1910.213(r), inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines.  On or about September 23, 2025, Hostetler Truss did not have adequate guarding to prevent employees from accessing the cutting blades on the Auto Omni Saw (component saw Serial Number A0970102).  Employees are exposed to the saw blade when in operation due to ineffective guarding exposing a gap in-between the 2 fence guards of approximately 14 inches as well as no protection over the top of the fence guard.
