As a result, employees were exposed to a fall hazard of over 14 feet.

As a result, employees were exposed to fall hazards of over 14 feet, while working at heights with personal fall harnesses attached to anchors at the feet and self-retractable lifelines that were not rigged to prevent a free fall of 6 feet or less.
