OFGL Construction LLC  at a job site located at  4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025, the employer did not maintain a safety and health program to ensure that fall hazards and struck-by hazards to the eyes were identified and corrected onsite while employees were performing framing work.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. OFGL Construction LLC  at a job site located at  4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025, the employer did not ensure that employees performing framing activities were protected from falling while installing floor joists. As a result, employees were exposed to a fall hazard of over 14 feet.

29 CFR  1926.502(d)(16):Personal fall arrest systems, when stopping a fall, shall:  29 CFR  1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.  a. OFGL Construction LLC  at a job site located at  4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025, the employer failed to ensure that employees were protected from fall hazards while installing floor joists on a newly constructed commercial structure. As a result, employees were exposed to  fall hazards of over 14 feet, while working  at heights with personal fall harnesses attached to anchors at the feet and self-retractable lifelines that were not rigged to prevent a free fall of 6 feet or less.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  b. OFGL Construction LLC  at a job site located at  4530 Generosity Way, Hilliard, OH 43026: On or about September 23, 2025, the employer did not provide a fall protection training program to ensure that employees engaged in framing activities at heights, were able to recognize the hazards of working at heights of over 14 feet.
