1926.454(c)(3)  The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  a) On supported scaffold, on the east side of a structure, located at 7227 FL-64 E., Bradenton, FL 34212; On or about 09/25/2025, the employer exposed employees to fall hazards of approximately 16 and 20 feet, in that employees did not retain information required pertaining to fall protection while conducting masonry work nor were they retrained.
