29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about October 2, 2025, during the installation of roofing material on a single story nursing home building located at 7187 St. Francis Street NE, Louisville, Ohio, the employer failed to ensure employees working on a 6:12 steeply pitched roof at a height of approximately 7.7 to 8.2 feet above the ground (eave height to ground), were protected from fall hazards, by utilizing a guardrail system, safety net system, and/or personal fall arrest system.
