29 CFR  1926.502(d)(6)(ii): Snaphooks not designed to be connected to one another were engaged to each other.  Site- A site supervisor working from the roof was not protected from a fall hazard of approximately 21 feet where their personal fall arrest system included the use of a retractable web lanyard and a second web lanyard connected to a rope grab and life line. The two web lanyards where connected to each other by their snap hooks to which they were not designed.

29 CFR  1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person.  Site- Employees working from the roof were not protected from a fall of approximately 21 feet where 2 employees were connected to the same anchor point, which was only rated for 1 person; the anchor point was not fastened to the roof utilizing all the required fasteners; and was not installed under the supervision of a qualified person.

29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:   Site- Employees working from the roof were not protected to a fall hazard where personal fall arrest systems were not inspected for wear, damage, and other deterioration. The retractable web lanyard used in the supervisors personal fall arrest system was damaged and the locking mechanism did not work, the retractable web lanyard was not removed from service. On or about 10/3/2025 a site supervisor fell approximately 21 feet from the roof and struck the ground while wearing a full body harness and connected to an anchor point.
