Site- A site supervisor working from the roof was not protected from a fall hazard of approximately 21 feet where their personal fall arrest system included the use of a retractable web lanyard and a second web lanyard connected to a rope grab and life line. The two web lanyards where connected to each other by their snap hooks to which they were not designed.
