OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck-by fragments of polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured: a. In the back of the facility at the RV repair/service area: On or about October 7, 2025 and at times prior, the employer exposed employees to the hazard of being struck-by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured: Shop Areas - PVC plastic piping was used in exposed above ground locations to transport compressed air at 110 psi exposing employees to struck-by hazards from shards of PVC if the pipe ruptures or explodes.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: guardrail systems; safety net systems; or personal fall protection systems: a. In the back of the facility at the RV repair/service area: On or about October 7, 2025, the employer exposed employees to fall hazards, in that employees need to climb on RV roofs over 10' to work on them without guardrail systems; safety net systems; or personal fall protection systems in place.
