29 CFR  1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   On or about October 9, 2025, at the single family residential home, where an employee was installing Christmas lights from the unprotected edge of the roof without any type of fall protection system in place.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  On or about October 9, 2025, the employer did not report to OSHA a work?related hospitalization prior to the lapse of the 24?hour reporting period after learning about the hospitalization. The incident was not reported to OSHA until October 13, 2025.
