29 CFR  1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)    At the transmission line pole behind the house, employees utilized an aerial lift at a distance less than 10 feet from an energized transmission line without implementing safety-related work practices to protect employees.  a) On or about November 1, 2025, an employee operated an aerial lift to mount a light bracket without checking the nominal voltages of lines and equipment in the area before work was started.  b) On or about November 3, 2025, an employee operated an aerial lift to reposition a light bracket without checking the nominal voltages of lines and equipment in the area before work was started.

29 CFR  1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)Note:  The work practices used by qualified persons installing insulating devices on overhead power transmission or distribution lines are not covered by 29 CFR 1910.332 through 29 CFR 1910. 335.  Under 29 CFR 1910.333(c) (2) unqualified persons are prohibited from performing this kind of work.   At the transmission line pole behind the house, employees working near an energized transmission line were not deenergized or grounded to protect from the risks of touching live electrical lines or equipment.  a) On or about November 1, 2025, an employee worked near an energized transmission line without utilizing grounding methods to mount a light bracket.  b) On or about November 3, 2025, an employee worked near an energized transmission line without utilizing grounding methods to reposition a light bracket.
