Location - Jobsite.  On or about November 11, 2025, an employee fell greater than 16' off a roof after disconnecting the lifeline from the roof anchor.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds per employee.   Location - Jobsite.  On or about November 11, 2025, fall protection equipment was not secured properly and capable of supporting at least 5,000 pounds in the following instances:  Instance A) Roof anchors not secured to the roof per the manufacturers requirements.  Instance B) Two lifelines attached to one roof anchor.  Instance C) Lifeline attached to a roof anchor with knot(s).
