OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were not protected from struck-by/ caught in between hazards.  Sunrise Office Services, Inc. Jobsite located at 177-01 Baisley Blvd. Jamaica NY.  a) On or about October 23, 2025, employees were exposed to struck-by/caught in between hazards while unloading boards from a box truck utilizing rolling A-frame carts and the truck's liftgate.  The employer failed to implement safe work practices including, but not limited to, training on the safe operation of the liftgate and other materials handling equipment   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  Sunrise Office Services, Inc. Jobsite located at 177-01 Baisley Blvd. Jamaica NY.  a) On or about October 24, 2025, the employer failed to notify OSHA within twenty-four (24) hours of a work-related incident that resulted in an employee hospitalization that occurred on October 23, 2025.   Note: Because abatement this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19
