29 CFR 1910.178(l)(3)(ii)(B): Powered industrial truck operators shall receive initial training in workplace-related topics; Composition of loads to be carried and load stability. At an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2: a) On or about November 19, 2025, and at times prior, employees were exposed to struck and crushed-by hazards created by employees operating two or more forklifts in tandem to lift and relocate steel skids from the facility to the outdoor storage yard. Employees had not received training regarding the load composition and load stability of tandem lifts. b) On or about November 1, 2025, and at times prior, employees were exposed to struck and crushed-by hazards created by employees operating forklifts while pulling skids on machinery skates out of the west bay. Employees had not received training regarding the load composition and load stability of lifting the skid with the forklift from one end while the other end rested on machinery skates.

29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline. On or about November 19, 2025, and at times prior, at an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2; employees utilizing forklifts to tandemly lift steel skids dismounted their forklifts with the load engaging means still elevated to support the skid. Employees had to dismount the forklifts with the forks still elevated to remove the rotary lifts from beneath the skid.

On or about November 19, 2025, and at times prior, at an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2; the employer did not ensure only safely arranged loads were handled when employees utilized two forklifts to tandemly lift and relocate steel skids from the facility to the outdoor storage yard. The skids were prone to shifting and falling off the forks when the forklifts had to turn or stop.

On or about November 19, 2025, and at times prior, at an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2; employees operating forklifts with rated capacities of 8,850 and 9,350 at 24 inches on center, were handling loads that weighed on average between 13,000 to 24,000 lbs. Employees utilized the forklifts to tandemly lift and transport fabricated steel skids from inside the facility out to the storage yard.

All items listed in (j)(2)(i)-(j)(2)(vii) shall be inspected for defects at intervals as identified in section (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard. On or about August 22, 2025, at an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2; employees were exposed to struck and crushed-by hazards created by falling suspended loads and a collapsing Marine Travelift 35BFM II mobile straddle crane when the hoist was defective and not taken out of service. Defects included, but were not limited to, the remote spontaneously disconnecting from the hoist's receiver and the front and rear hoist load gauges were broken and/or unreadable.
