At an establishment located at 2107 American Boulevard in De Pere, Wisconsin, Plant 2:  a) On or about November 19, 2025, and at times prior, employees were exposed to struck and crushed-by hazards created by employees operating two or more forklifts in tandem to lift and relocate steel skids from the facility to the outdoor storage yard. Employees had not received training regarding the load composition and load stability of tandem lifts.  b) On or about November 1, 2025, and at times prior, employees were exposed to struck and crushed-by hazards created by employees operating forklifts while pulling skids on machinery skates out of the west bay. Employees had not received training regarding the load composition and load stability of lifting the skid with the forklift from one end while the other end rested on machinery skates.
