29 CFR  1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   On or about November 14th 2025 Location: 370 Underhill Ave, Yorktown Heights, NY 10598  a) Employees were exposed to an approximately 20 ft fall while installing rails at the edge of the third floor balcony. Employees did not have fall protection system.

29 CFR  1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  On or about November 14th 2025 Location: 370 Underhill Ave, Yorktown Heights, NY 10598  a) An employee was exposed to an approximately 20 ft fall while standing on unopened step ladder while working as the edge of a balcony. The employee did not have fall protection system.
