29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s).  a) On or about September 24, 2025, at the warehouse facility, the employer failed to ensure that a powered industrial truck, found to be in a defective and unsafe condition, was removed from service. A Komatsu forklift, Model FG25ST-14, with Serial Number 590993A, was permitted to remain in operation despite the driver's rear left steer wheel emitting unusual noises, which was an audible indication of a serious mechanical defect. This incident therefore exposed employees to struck-by and crushing hazards.
