29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about November 19, 2025, employees were exposed to hazardous energy sources when performing maintenance and servicing, including, but not limited to, the Schaefer Glue Machine and the employer did not provide training.   The training shall include the following:   a.	Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  b.	Each affected employees shall be instructed in the purpose and use of the energy  control procedures.  c.	All other employees who work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  Employees who performed servicing and maintenance were not required to: (1)	Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2)	Turn off and shut down the machine,  (3)	Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4)	Affix lockout or tagout devices to each energy isolating device,  (5)	Prior to starting work on machines that have been locked out or tagged out, verify that isolation and reenergization of the machine or equipment have been accomplished.  (6)	Prior to starting work on machines that have been locked out or tagged out, verify that isolation and reenergization of the machine or equipment have been accomplished.   On or about November 19, 2025, an employee was exposed to ingoing nip points while performing servicing and maintenance activities, on the Schaefer Glue Machine because the employer did not cover the actions listed above and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 19, 2025, employees operating the Schaefer Glue Machine were exposed to caught-in and amputation hazards because effective machine guarding was not in place to protect operators from ingoing nip points.
