Paulus Excavating, Ltd., on or about November 19, 2025, at the site located near the NW corner of Bensman Rd. and Main St. in Minster, OH: The employer did not ensure employees were adequately protected from cave-ins while performing work to install a drainage pipe in an approximate 6.5 ft. deep unprotected trench.
